One-time Waiver in DSE
STUDENTS: As of Mon. 08/10, a one-time waiver in DSE must be completed prior to accessing Campus Recreation facilities or registering for Campus Recreation programs & services.
Step 1: Click profile button in top right and sign in using university credentials
Step 2: Once logged in, scroll down to Group X Classes and click the icon
Step 3: Read and sign the waiver

Beginning this fall 2026, members of the below sport clubs will be required to submit a sport preparticipation physical evaluation (PPE or "sports physical") by the first competition or September 30th, whichever comes first.

You may complete this PPE by scheduling an appointment with your own provider or at USC Student Health. This PPE is only required to be completed once during your time as a sport club participant at USC - you will NOT have to complete it again upon successful submission.
- If you choose to complete your PPE at USC Student Health, more information will be
provided prior to the start of the fall semester on how to schedule this appointment.

- If you choose to complete your PPE with your own provider, please print and bring
the USC Sport Club PPE form with you to your appointment - you MAY NOT use a different PPE/physical form than the one linked here.
- If you have had a sports physical completed within the last year, you may be able to request that record be transferred to the USC Sport Club PPE form and signed by the provider that completed your physical. You will still need to complete the health history portion of the PPE form (page 1) before submission.

How To Join
- All potential members are REQUIRED to complete a liability waiver on their club's DSE page prior to participation.
- Clubs are open to all current students (including Gamecock Gateway students), faculty and staff.
- If your club's practice location is an indoor CREC facility, you must have membership access.
